A completed and signed application form.
A copy of your district's reimbursement graduate school policy, if applicable.
A copy of your invoice.

If you are chosen, you will receive a form to be completed at the completion of the course. Reimbursement will take place only when the course is complete.If you have any questions, please email info@colleenritzer.org.
